9TH GRADE/TRANSFER STUDENT PHYSICALS

Illinois law mandates that all incoming freshman, out-of-state/country transfer students, and those coming from home school must submit the Certificate of Child Health Examination form. Sports physicals do not fulfill state requirements and will not be accepted. Parents must complete and sign the health history portion of the form or it is not considered valid.

SPORTS PHYSICALS

The Illinois High School Sports Association requires all students participating in interscholastic athletics to to have a certificate of physical fitness completed annually by a health care provider. To meet this requirement, fill out the IHSA Sports Physical form or have the health provider fill out the appropriate boxes on the Certificate of Child Health Examination form. Please note that the IHSA Sports Physical form is not acceptable for entrance into 9th grade or students new to the Illinois school system.

DISTRICT #157 AUTHORIZATION FOR ADMINISTRATION OF MEDICATION POLICY & PROCEDURE

All prescription medications administered at school will need the authorization of a licensed provider (MD/DO/NP/PA). There must be a current Prescription Medication Authorization form on file in the nurse's office with both the prescribing physician and a parent/guardian signature.

Over-the-counter medications may be dispensed in the nurse's office with a signed Non-Prescription Medication form.

All prescriptions, both prescription and over the counter, must be kept in the nurse's office and used under nurse supervision. The only exception is emergency medications, such as inhalers, epi pens, and insulin, which require the appropriate "self-carry" boxes addressed on the Prescription Medication Authorization form.

All medications must be in the original container and the student's name must be clearly written on it.
